What minutes are and how they differ from working notes is on the meeting minutes and meeting notes templates page; this page is the same record laid out as a file for Word.
What’s in this Word template
The file is one page of fields, in the order a reader looks for them:
- Organization header — organization name and address at the top of the page.
- Number, date and place of the minutes, on one line under the title.
- Meeting block — meeting; time from and to; chair; secretary; present; absent; invited.
- Agenda — a numbered list of the items.
- One block per agenda item — a numbered heading and three lines: Heard, Discussed, Decided.
- Action items — a table: number, action, responsible, deadline.
- Signature lines — chair and secretary, each with a line to sign and the typed name in slashes.

How to fill it in
One file per meeting, numbered in sequence, is what makes minutes easy to find a year later: the number and the date on the first line are how the record is referenced. Four rules come with the template:
- One file per meeting. Number minutes in sequence so they can be referenced later.
- Heard — Discussed — Decided for every agenda item; the decision is written in full, the discussion in two to five lines.
- Action items need a responsible person and a deadline; if none was set, write “not set”.
- Signatures make the minutes final. Keep the signed copy (scan or PDF) with the meeting materials.
Working with the template in Word
- Keep the tables. To add an action, insert a row into the existing table rather than drawing a new one, so the columns stay aligned from one set of minutes to the next.
- Agree the draft with Track Changes. Send the draft round with changes tracked, accept them once the group has agreed the wording, and only then print for signature.
- File the signed copy as PDF. Scan the signed page or save the final version as PDF, and keep the .docx as the working file for the next meeting.
The same minutes as a filterable log, one row per action, are on the meeting minutes template for Excel. For notes written by hand at the table rather than typed, there is a printable one-page meeting notes form.
Saving minutes as a Word file from a recording
You do not have to type the minutes from your own notes. Dalm is a Mac app that records a meeting — a call or a room — and drafts minutes from the transcript. Your speech is recognised on your Mac, and no audio leaves it; what goes to our servers is the transcript, as text, and a language model writes the draft from that text. The draft edits as plain text and saves as Word (.docx), PDF or Markdown — the header, the numbering and the signature lines of this template are still yours to fill in.

Questions
- Is this Word minutes template free?
- Yes. The .docx downloads with no sign-up and no e-mail address asked, and the PDF is free as well. The file is in English and carries no product name or branding, so you can put your own organization name and address at the top, change the fonts or delete the parts you never use.
- Does it work in Google Docs or Pages?
- The .docx opens in Google Docs, Pages and LibreOffice as well as in Word. Tables and headings usually carry over as they are; check the layout once after opening, because each program draws table borders and spacing a little differently. If it moved, the PDF shows what the page is meant to look like.
- How do I add more agenda items?
- Copy the block for one item — its heading and the three lines Heard, Discussed, Decided — and paste it below the last one, then change the number. Add the item to the agenda list at the top in the same place, so the numbers match, and add a row to the action table if the item produced an action.
- Who signs the minutes?
- In this template, the chair and the secretary, on the two lines at the end; the names in slashes are typed, the space before them is for the signature. Signatures make the minutes final. Whether your organization needs signed minutes, and who has to sign them, is set by its own rules; this is a template, not legal advice.
- Can I save the minutes as PDF?
- Yes. Once the minutes are filled in, save a copy as PDF from Word, Google Docs or Pages, and keep that copy with the meeting materials. A PDF is what you circulate and what you file, while the .docx stays the working file for the next edit.
